TAME (Targeting Aging with Metformin) The TAME trial is often called the "Holy Grail" of geroscience, not just for the drug (Metformin) but for the regulatory precedent it seeks to set. • The Core Goal: TAME isn't trying to prove Metformin is a miracle drug; it’s trying to prove that "Aging" can be a treatable medical condition in the eyes of the FDA. Currently, the FDA only approves drugs for specific diseases (like cancer or heart disease). TAME aims to show a single drug can delay a "cluster" of age-related diseases. • 2026 Status: As of early 2026, TAME remains the most influential trial that hasn't fully happened yet. While it received FDA "green light" status years ago, funding the massive $50M+ required for a multi-center, 3,000-person trial has been the primary hurdle. The TAME trial will use a composite primary endpoint that measures the time until a participant develops any one of a "cluster" of age-related conditions, including cardiovascular disease, cancer, dementia, or death. By grouping these together, the study aims to prove that a single intervention can delay multiple chronic diseases simultaneously. Additionally, secondary endpoints track functional markers like walking speed and biological "clocks" to see if the drug preserves physical independence and slows the underlying rate of aging. • Current Evidence: Recent smaller trials like MILES (Metformin in Longevity Study) and MET-PREVENT (2025/2026 data) have shown mixed results. While Metformin improves certain "hallmarks of aging" (like inflammation), it didn't significantly improve muscle mass or walking speed in already-frail seniors. This suggests it might be better as a preventative than a rescue therapy.
