What happens when researchers take nearly 300 people, follow them for 18 months, and compare a healthy diet with a Mediterranean diet and a high-polyphenol Green Mediterranean diet? They didn’t just measure weight. Researchers looked at the brain, visceral fat, blood sugar regulation, vascular aging, DNA methylation, and gene expression—which is why the DIRECT PLUS trial has become one of the most fascinating nutrition interventions I’ve read. The Green Mediterranean diet built on a traditional Mediterranean diet by emphasizing: 28 g of walnuts daily 3–4 cups of green tea daily 🍵 A daily Mankai (Wolffia globosa) green shake More plant-based foods Less red and processed meat Together, the green tea and Mankai added roughly 800 mg of polyphenols per day beyond the polyphenols supplied by the walnuts. In this video, I break down several studies that came from this same 18-month randomized intervention, including: • Brain aging: In adults 50+, the Mediterranean and Green-MED interventions attenuated age-related changes in hippocampal occupancy, with the Green-MED group showing the most favorable outcomes. Improved insulin sensitivity was strongly associated with less brain atrophy. • Metabolic health: A later analysis found that improvements in HbA1c, fasting glucose, and insulin resistance were associated with more favorable brain-aging measures—and greater Mankai and green tea intake was associated with those effects beyond weight loss. • Visceral fat: The Green-MED group reduced visceral adipose tissue by about 14.1%, compared with 6.0% in the traditional Mediterranean group and 4.2% in the healthy-diet group—even though overall weight loss between the two Mediterranean groups was fairly similar. • Vascular aging: Proximal aortic stiffness decreased by roughly 14% in the Green-MED group, compared with about 7.3% with the Mediterranean diet and 4.8% with healthy dietary guidelines. • Epigenetics: Researchers found substantially more DNA-methylation and gene-expression changes in the Green-MED group, including 1,573 differentially methylated regions and 1,753 differentially expressed genes in this analysis. What I love about this research is that the takeaway doesn’t have to be, “Completely overhaul your diet tomorrow.” You could simply start adding more polyphenol-rich foods consistently—walnuts, green tea, berries, leafy greens, herbs, spices, extra-virgin olive oil, pomegranate, cocoa and other deeply pigmented plants. Nutrition isn’t just about calories or weight. Food is information. What we eat can influence metabolic signaling, our microbiome, inflammation, vascular function, brain health—and potentially even how our genome is regulated.
