Irritable Bowel Syndrome (IBS) is a neurogastrointestinal disorder affecting 10%–15% of adults. It is characterized by communication failures between the brain and the gut, leading to chronic discomfort without visible tissue damage.

The Three Main Types
Diagnosis is based on stool consistency during flare-ups:
• IBS-C: Predominant constipation (hard/lumpy stools).
•
• IBS-D: Predominant diarrhea (loose/watery stools).
• IBS-M: Mixed habits (alternating between both).
Why It Happens (Biological Mechanisms)
• Dysmotility: Abnormal muscle contractions in the colon.
• Visceral Hypersensitivity: Extra-sensitive gut nerves with a lower pain tolerance.
• Gut-Brain Axis: Communication errors that make the gut reactive to stress (often called "nervous stomach").
• Microbiome Changes: Altered gut bacteria and history of severe infections or childhood stress.
Diagnosis and "Alarm" Symptoms
IBS is diagnosed via the Rome IV criteria (pain related to bowel movements at least once a week). Providers must rule out "organic" disease if alarm symptoms appear:
• Rectal bleeding, fever, or anemia.
• Unexplained weight loss.
• Diarrhea that wakes you up at night.

Management Strategies
Because there is no "cure," treatment focuses on long-term symptom control:
• Dietary Changes: The Low FODMAP diet. FODMAP is an acronym for a group of short-chain carbohydrates (sugars) that the small intestine often has trouble absorbing. For most people, these sugars are harmless, but for those with a sensitive gut or IBS, they can trigger significant distress. (reducing fermentable sugars) and increasing soluble fiber (ispaghula/psyllium).
• Pharmacotherapy: * Pain: Antispasmodics (Dicyclomine) or low-dose antidepressants (neuromodulators).
o Bowel Habits: Secretagogues for constipation (Linaclotide) or Rifaximin for diarrhea/bloating.
• Mind-Body Therapy: Cognitive Behavioral Therapy (CBT) and hypnotherapy to desensitize the gut-brain axis.
• Lifestyle: 150 minutes of weekly exercise and 7–9 hours of sleep to reduce "nervous gut" triggers.
