About this video

Creator description · original textText language: English

Ask your clinician for a fasting plasma total homocysteine. Bring the number, not just "normal." If it is elevated, discuss methylated B vitamins (folate / B12 / B6 forms) with them. Do not start high-dose B vitamins from this tip alone. Educational tip only. The lie I taught in medical school: if the lab says your homocysteine is "normal," you can ignore it. Reference ranges often call under about 15 µmol/L normal, while research risk and treatment response show up well below that ceiling. Many clinics stopped ordering the test after secondary-prevention B-vitamin trials looked "negative." Smith AD et al., PLoS ONE 2010;5(9):e12244. VITACOG. Single-center RCT, double-blind. 271 adults over 70 with mild cognitive impairment randomized; MRI completers n=168 (85 active, 83 placebo). Active: folic acid 0.8 mg/d + vitamin B12 0.5 mg/d + vitamin B6 20 mg/d for 24 months vs placebo. Mean whole-brain atrophy per year: 0.76% (95% CI 0.63-0.90) active vs 1.08% (0.94-1.22) placebo (P=0.001), about 30% slower atrophy on active treatment. In those with baseline homocysteine greater than 13 µmol/L, atrophy rate was 53% lower on active treatment (P=0.001). No difference in serious adverse events by treatment category in the report. DOI 10.1371/journal.pone.0012244 PMID 20838622. MCI population over 70. Not a healthy-young primary-prevention trial. Not proof that testing alone stops Alzheimer's or heart attacks. Large secondary-prevention B-vitamin trials for major vascular events often showed little effect on MI/death over about 5 years; stroke signals and brain-imaging findings are a different question. Do not claim B vitamins prevent heart attacks. Forms, dose, kidney function, and fortification context matter. Educational tip only. Not medical advice. Do not start, stop, or change any therapy from this reel.
