I went through 50+ studies on intermittent fasting. Rated each across three things: peer-reviewed, RCT, sample size (no science background? read the pinned comment first….I’ve explained all three in normal-human English).. here are the main findings that emerged out of that analysis
1. The "IF kills you" headline was never peer-reviewed.
A conference presentation, not a published paper. Based on people remembering what they ate over 15 years - twice. Only 414 of 20,000 were doing IF. 34 researchers signed an open letter saying it should never have been announced.2. The anti-ageing magic? Almost all on mice.
2. Autophagy is the most repeated claim in IF marketing. Strong evidence in mice. Human muscle biopsies didn't show the same activation. The one human study with a signal had 121 people.3. The fasting window isn't doing anything special metabolically.
3. RCTs gave one group IF and another the same food spread across the day. Same calories, different windows. Identical results. The window helps people eat less. That's the mechanism.
The TREAT trial (JAMA Internal Medicine, 2020).116 obese adults. 12 weeks. IF group: 1.17%. Control: 0.75%. Statistically the same. The paper's own conclusion: "Time-restricted eating did not confer weight loss or cardiometabolic benefits."
4. NEJM 2022 confirmed it long-term.139 patients. 12 months. TRE + calorie restriction vs calorie restriction alone. No significant difference. When calories matched, the window stopped mattering.
5. The one finding that held up: meal timing….Two Cell Metabolism RCTs : people who ate earlier had better insulin sensitivity and lower blood pressure. Same food. Different timing. The real finding isn't the window. It's the clock.
6. Women: more nuanced than the internet says. The "IF wrecks female hormones" narrative comes mostly from rodent studies on rats equivalent to 9-year-old humans. Human trials show standard 16:8 has minimal effect on reproductive hormones in young women. Risk lives at the extreme … OMAD, chronic breakfast-skipping, or fasting through the luteal phase. One nuance for Indian women: early evidence suggests TRE may actually help PCOS. Moderate, cycle-aware. Not extreme.
7. The honest verdict.
IF works but because it helps you eat less without tracking. The anti-ageing magic isn't proven in humans. The death scare was never peer-reviewed. If IF works for you - keep going. If not - shift your bigger meals earlier. Same benefit. No window required.
