What single number best predicts how long you'll live?
Not your cholesterol. Not your blood pressure. Not even your weight.
It's your VO2 max — the peak volume of oxygen your body can use during exercise. And most physicals never test it.
The evidence: Mandsager et al. tracked 122,007 adults over a decade (JAMA Network Open, 2018). Low VO2 max was more strongly associated with mortality than smoking, diabetes, or high blood pressure. Elite fitness was linked to a 5-fold lower risk of death vs. the bottom quartile.
Honest caveat: this is observational — it shows association, not proof of cause. But the signal is consistent across dozens of cohorts and it's mechanistically plausible.
As a medical school professor who has worked at both UCLA and USC, I now teach clinicians to think of VO2 max as a vital sign. It reflects heart, lung, muscle, and mitochondrial function all at once — the metabolic engine of longevity.
You can move up the curve at any age. Zone 2 training + short high-intensity intervals is the fastest proven way to raise it.
Ask your doctor for a CPET, or use a validated wearable estimate. Track it. Train it.
I'm Dr. Robert Lufkin, and that's how you live well, longer.
